In short: A night guard protects the teeth from wear, but it does not stop the grinding. Bruxism usually arises from a combination of stress, sleep quality and uneven bite loading. So only the combination works: the guard as protection, plus work on the causes. Without the second part, the guard wears down instead of the teeth.
Most people who grind do not know it from themselves. They hear it from a partner, or in the practice, because the chewing surfaces tell what happens at night.
The signs
- Tense jaw muscles in the morning, pressure in front of the ear, sometimes headache at the temples
- Flat, polished-looking areas on the chewing surfaces and front teeth
- Sensitive teeth without visible decay; chipped enamel edges
- Clicking or grating in the jaw joint, limited mouth opening
- Repeatedly broken fillings, cracked ceramic, crowns that come loose
What is actually happening
Chewing applies short, measured forces. Nocturnal clenching and grinding applies far higher forces over long periods, without the protective reflexes of the waking state. The consequences hit three levels: the tooth itself, which loses substance; the jaw joint and muscles, which are overloaded; and every restoration in the mouth — ceramic takes a lot, but not unlimited amounts.
For us this is also a planning issue. Anyone who grinds and receives an implant or veneers needs a different material choice and an accompanying guard. Ignore that and the restoration will not last.
What the guard achieves — and what it does not
An individually made night guard distributes the forces, separates the rows of teeth and relieves the muscles. It is the most effective protection against further wear and is almost always worthwhile. What it does not do: remove the cause. The grinding continues, just not on your teeth.
We advise against shop-bought or self-fitted guards. If a guard does not fit exactly, it shifts the load instead of distributing it and can worsen joint symptoms.
The causes that must be treated alongside
First is the strain situation in daily life. Bruxism is well documented as a response to sustained tension, and no dental procedure changes that. What works is relaxation techniques, physiotherapy for the chewing muscles and, where indicated, a sleep assessment — snoring and breathing pauses are linked to grinding. On the dental side we check whether the bite contains interfering contacts that force the muscles into an unfavourable position, and correct those specifically.
The honest expectation: bruxism is usually controlled, not switched off. The goal is a state where the teeth are protected and you wake up without symptoms.
Frequently asked questions
Does German insurance cover a night guard?
With a corresponding diagnosis a guard is generally covered by statutory insurance. More elaborate versions or an accompanying functional analysis may be private and are discussed beforehand.
How long does a guard last?
Depending on the force and frequency of grinding, several years. Visible wear on the guard is a good sign — it would otherwise have been on your teeth.
Can orthodontics fix grinding?
A well-ordered bite can distribute load more favourably. As a treatment for bruxism on its own it is not established; the causes usually do not lie in tooth position.
Do children grind too?
Yes, and in the mixed dentition that is often harmless and temporary. It becomes notable with heavy wear, pain or joint noises — then it should be examined.
