New in Düsseldorf or changing practice

Dear patient,
changing practice is straightforward — you need no referral and no justification. Here is what to bring and how to obtain your existing records.

Please bring with you
  • Electronic health card
    for statutorily insured patients. If you are privately insured, please bring your insurer details.
  • Passport or national ID card
  • Bonus booklet
    It is real money: attending the annual check-up without gaps earns you a higher fixed subsidy towards prosthetics later.
  • Radiograph, allergy, anticoagulant or implant card
    if you have them.
  • A current list of your medication
    including supplements and herbal preparations.
  • Completed registration form
    You can download it in advance at www.aplus-dental.de. It noticeably shortens your first appointment.

Records from your previous practice

Request
You are entitled to copies of your treatment records, including radiographs. A short written request to the previous practice is enough.

Allow time
A reasonable charge may be made for the copies. You do not have to give a reason for changing practice.

Or start fresh
If you would rather avoid that: we examine you afresh and take the images we need ourselves.
If you are new to Germany
  • Statutorily insured patients present the health card once; check-ups and pain treatment are covered.
  • Privately insured patients and self-payers receive a cost estimate in advance.
  • Supplementary dental insurance is best taken out before treatment is needed — existing findings are usually excluded.
Languages & getting here
  • We treat in German, Spanish, English and Portuguese. Tell us your preferred language when booking.
  • Cheruskerstraße 111, right at Belsenplatz. Tram to the Belsenplatz stop.
  • The practice is on the second floor and is step-free via the lift.
