After plastic periodontal surgery
and peri-implantitis surgery

Dear patient, the result is decided over the next two weeks. During this time your cooperation matters more than any further procedure.

The three most important rules
1 · No toothbrush in the wound. The surgical site is not cleaned mechanically for 14 days.
2 · Do not pull your lip. Lifting cheek or lip to check the sutured area.
3 · Sutures stay in for 9–11 days. Please keep the appointment exactly.
The first 24 hours
  • Cool from the outside, several times a day for 10–15 minutes, using a cold pack wrapped in a cloth.
  • Do not rinse, suck or spit. Negative pressure dislodges the blood clot.
  • Wait to eat until the anaesthetic has worn off completely.
  • Sleep with your upper body raised. No sport, no sauna, no hot baths.
Medication
  • Ibuprofen 400–600 mg, up to three times daily with at least six hours between doses. Take the first dose before the anaesthetic wears off.
  • No aspirin / ASA — it promotes bleeding. If you have stomach or kidney problems we will switch you to paracetamol.
  • Finish any prescribed antibiotics completely.
Chlorhexidine – your toothbrush for now
  • From the day after surgery, rinse twice daily with CHX 0.2 % for one minute each time.
  • At first do not rinse vigorously — move the solution gently or dab it on.
Eating and smoking
  • Chew on the opposite side. Soft, lukewarm food; nothing crumbly or sharp-edged.
  • No drinking straws, and no alcohol while you are taking medication.
  • No nicotine. It constricts the very vessels the graft lives on — the single strongest risk factor for a poor result.

How to brush the surgical site

into the wound

The bristles are placed into the gum margin and vibrated there. That works its way under the flap, opens the suture and pulls the graft away. Interdental brushes and floss also stay away.

coronal apical

Place the bristles on the gum and sweep them towards the crown in a soft rolling motion, from apical to coronal — upper jaw downwards, lower jaw upwards. Always away from the wound edge.

Your care plan
Days 1–14: do not brush the surgical site at all, CHX only. Carry on brushing the rest of your teeth normally.
Days 9–11: suture removal. Then start an ultra-soft surgical toothbrush and the rolling motion.
From week 4: interdental cleaning once we have cleared you for it.
After that: check-ups at 4 weeks and 3 months, then maintenance every 3–6 months.
What is normal
  • Swelling peaking on day 2–3, then subsiding.
  • A whitish film on the wound — that is fibrin. Do not remove it.
  • A feeling of tension and teeth that look longer at the surgical site.
Please contact us immediately
  • Heavy bleeding that does not stop after 20 minutes of biting down.
  • Increasing pain or swelling from day 3, fever above 38 °C, pus.
  • A loosened suture or exposed graft.
